Permission forms for use at Youth Ministry events sponsored by a parish youth group, or event organizer. Parents do not download these forms and fill out if it is blank. An event organizer MUST fill out the top portion of the form before parents sign.

Form A – Parental Consent Liability Waiver and Medical Form – for Minors

Form B – Adult Consent Liability Waiver and Medical Form – for Adults

Form C – Volunteer Driver Form – This form is for those people who will be driving youth to and from parish/diocesan events. All drivers must be 21 years old and have minimum insurance coverage listed on the form.

A new form is to be filled out for every event.
